YOUR DS-2019

USC is issuing you the enclosed DS-2019 document so you can apply for a J visa at a U.S. embassy or consulate.  Be sure to carry your DS-2019 and all other immigration documents on your person when you travel to the U.S.  Do not pack them in your luggage.  Please note that if you use the DS-2019 form issued by USC to enter the U.S., U.S. immigration regulations require that you report by the start date.  The earliest you may enter the US is thirty days before the reporting date listed on the DS-2019.  Once you have reported to USC you must enroll and attend classes for the current semester. The following explanations are listed out in the order that the information appears on your DS-2019:

1. Student Information:   It is important that this information matches the information that is on your passport.

2. School Information:
This lists the name of the school that you will be attending once you are granted your J-1 status. Please note that you must enroll and attend classes at the school listed on the DS-2019 that you used to enter the country.

3-4. Student’s Purpose: This information is specific to your program of study. The information reports what degree level you are pursuing, the major you are studying, and the estimated length of time you will need to obtain your degree.

5. Financial Certification: This information documents the total expenses for the program and how much funding you are estimated to receive. If there is an amount listed as “personal funds,” you should be prepared to show the visa official a bank statement for that amount.

6. State Notification: This is completed by the visa official.

7. Responsible Officer: This section is completed by U.S.C. and must be signed before you apply for your visa.  
8. Transfer of Program: This section is only applicable if you are transferring from one program to another within the United States.

212(e) Requirement: This section will be completed by the visa officer at your visa appointment. He or she will indicate in this section whether or not you are subject to the 212(e) 2 year home residency requirement (see page 2 of your document for more information.)

Travel Signature: The issuing officer from your university or program sponsor is required to sign this section if you decide to travel outside the U.S. during any period of your studies after your first entry in the United States.

Required Signatures: Please read page 2 of the DS-2019 which explains your requirements under the J visa. After reading page 2, you must sign your name and date at the bottom of the DS-2019, page 1.
