USC Program Application Materials
Following is a checklist to help
you maneuver through the application and post-acceptance
process. Please click on the links below to access the forms
required for the USC Program Application. All forms must be
typed, then printed, and submitted to the Study Abroad
Office before March 1 for Fall and Academic Year
programs OR before October 1 for Spring programs. An
administrative fee will be applied to all late applications. Where
links aren’t available, please stop by the study abroad office
to obtain the required forms.

Study Abroad Applications received after
5:00pm on the day they are due must be accompanied by a $30
Administrative Fee, by check only, made payable to USC. |

Two Passport Photos for Host Institution
Once You have been Accepted,
Complete the:
Placement Acceptance Form
Host Institution Application/Accommodation Forms (varies by
institution)
Medical Self Assessment Form
Press Release Form
Liability/Waiver Form
Financial Aid for Study Abroad Pre-Screen Form (T1 ) (submit to
Financial Aid Office)
Then Register for Study Abroad by:
Submitting a copy of your
Interntaional Student Identity Card
(required for emergency medical coverage)
Submitting the Study Abroad Approval Form (signed by all
parties) (sample form)
Gaining Authorization to Register for the appropriate INTL
placeholder course.
