Alternative Format Textbook Request

In order for the Office of Student Disability Services to obtain your books in alternative format you must fill in the following information completely. This will help ensure your books will be ordered in a timely manner. It often takes a minimum or 4-6 weeks to obtain a book.

Steps for Ordering Books

Contact your professor or look on the USC Bookstore website

http://sc.bkstore.com/ to find which books will be needed for each
class.

Once you find out the titles of the books, call the bookstore (803)777-4160 to get the complete information for this form (see what information is needed below)
Once you have the information, complete the form and click submit.
