Test proctoring request form

 

You must schedule your exam with our office at least one full week (seven days) before the exam. You should schedule your exam as close as possible to the day and time the class is taking the test. The test proctor will email you a confirmation and let you know if there are any scheduling conflicts.

Our testing hours are flexible, but contingent on staff availability.

Pick up a Test Proctoring Envelope from our Student Services Office in room 112A LeConte College. It is your responsibility to complete Section I of the Proctoring Form and deliver it to your professor at least five days before your appointment.

You must arrive on time for your proctoring appointment. If you are more than 20 minutes late to a scheduled exam, the proctor will write “No Show” across the top of the Proctoring Form and return it to our Administrative Office in room 112B LeConte for faculty/designee pick up. Permission to reschedule will be granted only at the faculty member's request.
