A student in
your course is eligible to have exams proctored through the
Office of Student Disability Services. The student has been
informed of their responsibilities and the procedures to use
this service. Some faculty members choose to provide special
testing accommodations themselves, while others choose to have
our office proctor the tests for them.
If our office
is used for testing, it is the student's responsibility
to schedule the test day/time with our office at least one full
week (seven days) before the test. All tests must
be scheduled via our website at www.sa.sc.edu/sds/.
All we ask of
you...
Ø
Before each exam, the student should provide the professor with
a Proctoring Form, attached to an envelope, which is to be
filled out completely. Please bring the exam to LeConte
College room 112A by 5:00pm the day before the scheduled
exam. The proctor will return the exam to room 112A in LeConte
for faculty/designee pick up, unless instructed otherwise.

Students should take their exams on the same day and time as the
class, when possible. Our hours are flexible, but
contingent upon staff availability. If your class
does not meet within our time parameters, and you are not able
to provide the accommodations yourself, please let the student
request testing accommodations during a time within our hours
that you approve of. If you have any questions, please

If the student is more than 20 minutes late to a scheduled exam,
the proctor will write “No Show” across the top of the
Proctoring Form and return it to room 112B in LeConte for
faculty/designee pick up. Permission to reschedule will only be
granted at the faculty member's request.

Exams that must be enlarged or brailled for visually impaired
students must arrive in our office at least three days before
the proctoring appointment.
Ø
Faculty members who anticipate assigning exams within less than
one week’s notice, and choose to have the exam proctored, should
contact our office.

procedures on the back of this document.

Exams are filed in a locked cabinet in our Administrative Office
(LeConte College 112B).
Ø
Students are monitored by cameras and/or a test proctor in our
proctoring room.
Procedures for Faxing or Emailing an Exam
If faxing of
exams is desired, please adhere to the following procedures:

Please fax the exam to 777-6741 by 12:00 noon on the day before
the exam.
ü
Please fax every exam with a completed proctoring form as a
coversheet.
ü
Please call 777-6142 to ensure that your exam was received.
If emailing of exams is desired, please adhere to the
following procedures:

Please email a WORD or PDF attachment to
SAPrctr@mailbox.sc.edu by 12:00 noon on the day before the
exam.
ü
Please include specifics in the email including your name and
phone number, the student's name, name of class, date/time of
test, how much time the class is allowed, and any other specific
directions.
ü
Also include whether you plan to pick up the exam, have the
student return it to you, or would like it returned by fax. If
by fax, then please be sure to include the fax number where the
exam should be returned.
