Wheelchair Accessible Desk Request:

**Visit your classrooms to determine whether or not you will need an accessible desk.    If a desk is needed, use this form to place your request.    

About 1 month before classes start our office places a request with Facilities.   If you are placing this request just before classes begin, please be aware that while the desks are placed in classrooms as soon as possible, they may not be in your classrooms the first week of classes.

Desks that roll up and down are placed in rooms when possible.    If Facilities runs out of roll desks, they will place a table in the room.  

If you change any classes, please be sure to contact our office.

If you have any problems with your requested desks, please contact our office.    Otherwise we will not know about the problem.
