____VA Application Form (Chap. 30 - Form 22-1990) *

____l Copy of DD214 - certificate of release or discharge

____USC Enrollment Certification Request Form 

*A "Certificate of Eligibility" from the VA can substitute for VA form 22-1990 and the DD214.  Also, if Chapter 34/30 conversion ask a Veterans Services’ Staff about other necessary forms 21-686c & 21-674.
